Authorization for Release of Medical Information
To request copies of your medical records, please fill out the Request for Release of Medical Information form. Download a form HERE.Completed forms can then be faxed to Benefis Medical Records at 406.455.4998 or mailed to:

Advanced Directives
No one likes to think about the end of life, but having the discussions with a spouse, significant other or relatives can help you plan so that death occurs according to your wishes. This planning should include putting your wishes in writing and assigning a specific person or persons to make healthcare or final decisions for you when or if you are not able to make such decisions yourself.
