We realize a hospital stay can be stressful and overwhelming. Our dedicated team work collaboratively (physicians, nurses, technicians, dietitians, volunteers, administration and members of the support staff) to help make your stay as comfortable as possible.

The following forms and documents will be beneficial to you, should you need to get information from your medical record or wish to provide care information to your family and medical caregivers.

Authorization for Release of Medical Information

To request copies of your medical records, please fill out the Request for Release of Medical Information form. Download a form here.Completed forms can then be faxed to Benefis Medical Records at 406.455.4998 or mailed to:

Advanced Directives

No one likes to think about the end of life, but having the discussions with a spouse, significant other or relatives can help you plan so that death occurs according to your wishes. This planning should include putting your wishes in writing and assigning a specific person or persons to make healthcare or final decisions for you when or if you are not able to make such decisions yourself.

Email a Patient

Welcome to Benefis Hospitals' patient email service. This is a new way to send your thoughts to family and friends during their stay with us. Simply complete the form below, including the patient's first and last name and your personal message. Submit the form and we will print and deliver your best wishes to the patient within 24-hours on weekdays.

Emails are delivered by Benefis Volunteers Monday through Friday starting at 10 a.m. Emails coming in after 10 a.m. will be delivered the next morning. Emails that come in during the weekend will be delivered the following Monday morning.

Please Note: We cannot deliver patient email to patients in our Emergency Room, patients who are receiving outpatient surgery or diagnostic testing. The content of your patient email is not confidential and will be seen by members of the Benefis Volunteers.

Messages received after a patient is discharged will be mailed to the patient's home.
