Registration Guidelines & Form

If you would like to borrow materials from the SCI Resource Center, please read the guidelines and complete the registration application below.

Use of the SCI Resource Center:

• SCI Resource Center is open to the public 8 a.m. to 5 p.m., Monday through Friday.
• Circulating materials may be borrowed free-of-charge by patrons who have a completed application on file.  No application is required for in-house use of materials while at the SCI Resource Center.

 

Borrowing Materials:

• To better maximize public access to cancer-related materials, no more than five (5) items may be checked out in a patron's name.
• Circulating books may be borrowed for 21 days.
• Circulating audiotapes, videotapes, and/or CD's may be borrowed for 10 days.
• Electronic equipment (i.e., DVD, CD Player, MP3) must be returned before 5 p.m. on the same day borrowed.
• Reference books and materials, newsletters, and magazines do not circulate (please note – the high cost of some materials prohibits us from making them available for loan.  Feel free to ask our staff members for information about using these materials while at the SCI Resource Center).

Renewals:

The due date of an item may be extended by visiting the SCI Resource Center or by calling us at (406) 731-8225.  If another patron has requested the material, it cannot be renewed and must be returned by its due date.  Items in the SCI Resource Center may be renewed only if no one else has requested them.

Overdue Materials:

If items are not returned by their due date, you will not be able to borrow additional materials from the SCI Resource Center until they have been returned.  We will notify you of overdue materials within two weeks of the item's due date.  If the item has not been returned after four weeks, you may be charged the replacement cost of the item(s).

Lost or Damaged Materials:

You are responsible for all materials checked out in your name.  Please return books and materials in the same condition in which they were received.  If you lose an item you have checked out, or if the material becomes damaged while in your care, you may be assessed the cost of the material.
