Ways to Access Your Medical Records

MyChart is a free online portal offering personalized and secure access to your medical records. This is the fastest way to access your records.

Patient Portal

Request Electronic Copies of Your Medical Records

Submit an electronic request using the button below. Please have a current photo ID ready and ensure your device has a camera. We recommend using Chrome, Firefox, or Safari web browsers. The form should take five to 10 minutes to complete.

At this time, we are unable to accept online requests that contain an international mailing address. Please refer to one of the other available options for requesting records.

IMPORTANT: Please complete the authorization form in its entirety. You must send a copy of your valid driver's license, passport, or other government/legal form of identification. This is required to properly identify you and protect your privacy.

Consent for Verbal Release

Request Your Records In Person

Medical Records is located on Benefis East Campus in the North Tower and is open for on-site records requests and pickup services from 8 a.m. to 4:30 p.m. Monday through Friday. Please be sure to bring a valid ID.

For Third Parties

Benefis fulfills third-party record requests electronically via ChartSwap, a HIPAA-compliant platform launched to facilitate electronic medical and billing record exchange between medical providers, law firms, and other requesting parties. It’s free to register, and requestors can use the system to request, track, pay for, and download records.

Register for ChartSwap

From there, you can log into your ChartSwap portal directly to search for a provider, enter request details, and upload supporting documents.

Once your request has been reviewed and records are available, you will receive a notification and invoice that you can pay with a check or credit card.

Please note: The fastest way to obtain your medical records is by signing up for our patient portal. Under HIPAA, your provider may not obtain medical records from us without an authorization.
